Equipment Return
and Shipment Policy

This document (“Equipment Return and Shipment Policy”) describes the equipment return and shipping policies of residential and business high-speed internet services (“Service” or “Services”), provided by Lakeshore Fiber (“LSF”).

In-Home Equipment Return:

Within 15 days of the termination, all equipment provided by LSF for in-home LSF service will be returned to LSF by the Person listed on the LSF account, using the “Lakeshore Fiber Equipment Return Procedure”. In the event that the Equipment is destroyed, damaged (with the exception of damage caused by Acts of God), lost or stolen or the inside Equipment is not received by LSF for any reason within 30 days of termination, you will be liable to LSF for the full replacement cost for any unreturned or damaged Equipment. Further, you understand and agree that LSF may charge the credit card on file at time of termination of Service for the cost for any unreturned or damaged Equipment, in accordance with applicable law.

Shipment of Equipment from LSF

Important: Please make sure your identified receiving person of contact and receiver along with address is correct before shipment occurs as the signed contract will be followed regarding time frames and shipment information. LSF will not be responsible for incorrect addresses or non-named persons. We make every effort to ship your equipment safely and quickly and include insurance and tracking with all shipments. Signature confirmation available upon request. Ownership of packages turned over to USPS, Fed Ex, or UPS transfers to the customer named on contract. We are not responsible for lost, held, forwarded, transferred, misplaced, or damaged packages. We are not responsible for mis-delivery errors via carrier or incorrect shipping info. Any shipping issues must be handled by the provided tracking carrier. Additional insurance is available to add to any shipment. We are responsible for your equipment until it reaches the carrier location and is scanned in. After that, it is out of our control. Insurance is to protect against loss or damage during shipping. In the case that your shipment is lost or damaged in transit, you are responsible for filing the claim through the given carrier. We are not responsible for lost packages or for shipments sent to forwarding services, old addresses, or any non-deliverable addresses due to new builds or move ins. Once a package is confirmed delivered via tracking, receiving customer is fully responsible and liable for entire retail value and contents of package and postal insurance cannot be claimed. Price and costs of shipping are either covered by LSF, or by customer in pre-determined situations depending on rate of speed and emergency. Please make sure that you triple-check the contact person and address and notify us of any special requests in advance as the signed contract will be followed.
